The Supportive Housing Program provides GRH rental assistance and supportive services for individuals living in Hennepin County. Program participants choose their own apartment in the area they want to live. Participants must have a documented disability. ReEntry House staff specialize working with individuals who have been diagnosed with a mental illness or co-occurring disorder
of mental illness and chemical dependency.

Program staff assist
participants in obtaining an apartment, furnishings, and community support in order to live independently in the community.  ReEntry House also helps clients manage their finances such as rent, food, and utilities. Program participants receive a monthly amount for personal needs and are eligible for SNAP benefits. We have two supportive housing options – GRH Demo and LTH (Long-Term Homeless).


  • Must have a documented disability – primarily a diagnosis of a mental illness.  Many of our clients also have issues with Chemical Dependency.
  • 18 years or older with no children in their care
  • Low income (under 30% of the median income)
  • Meet eligibility for GA or SSI with limited assets
  • Participants of our LTH program must meet the Minnesota definition of Long Term Homeless (at least 1 year continuously homeless or 4 times in the last 3 years)

Referrals & Wait List Information:


  • GRH Demo – wait list is currently closed due to the long wait.
  • GRH LTH – all referrals must go through Hennepin County Coordinated Entry.  The current focus is on housing those who are staying at the shelter or outdoors.

    — Ask to speak with an advocate/staff at one of the local shelters about being assessed for supportive housing.
